The Medical Forensic Exam

The Forensic Center of Excellence responds 24/7 for medical forensic exams in Houston, North Texas, and the Rio Grande Valley. Medical exams are provided to over 100 locations throughout the three programs.

What is a forensic exam?

A forensic exam is for anyone affected by sexual assault, human trafficking, domestic violence, physical assault, strangulation, child abuse, or elder abuse. Conducted by a trained nurse, this exam gathers evidence for legal cases and meets the physical and emotional needs of the survivor.

7 steps to a forensic exam:

INFORMED CONSENT
The nurse will explain the exam process. The patient or guardian must agree to it, and the patient can refuse any part of the exam.

MEDICAL FORENSIC HISTORY
The nurse will review the patient's medical history and ask about the assault to provide appropriate care.

PHYSICAL EXAM
The nurse will examine the patient from head to toe, noting and photographing any injuries or findings.

ANOGENITAL EXAM (If Applicable)
The nurse will check the genital area and anus, documenting and photographing any injuries or abnormalities.

EVIDENCE COLLECTION (If Applicable)
Evidence can be collected from sexual assault patients for up to 120 hours after the assault, which may include swabs, clothing, and toxicology tests. An exam is still possible if the assault occurred outside this window.

MEDICATION AND MEDICAL TREATMENT
The nurse will suggest medications to prevent sexually transmitted infections, pregnancy, and HIV. Other treatments may be required and performed in the hospital.

SAFETY AND DISCHARGE PLANNING
The nurse will ensure the patient has access to follow-up care and advocacy resources.
