Concerns should first be voiced at the time, if possible. You may ask to talk to a supervisor at any time. It is our goal to treat each patient with dignity and respect and we are dedicated to providing excellent, patient- centered care.

FILING A COMPLAINT

If you would like to escalate your concern to the Office of the Attorney General you can download a complaint form at https://gov.texas.gov/organization/cjd and submit the form to GAD@gov.texas.gov. You may also contact the Office of /Civil Rights and Civil Liberties at (800) 421-3481 or email them at OCR@ed.gov. Directions for filing a complaint can be found at www.2.ed.gov.
